It's still the fans for sure. They could just be towards the end of their lifespan. I have a Legion Pro 5 and had to replace a fan a few months ago after only about 16 months of use mostly in performance mode, so I think that if you've had it since 2019 it's about time they did give out. The sound probably goes away and comes back because the fan speeds vary.
